Attributes That Nose Surgery Can Correct

Prospective Boston, Wellesley and Sudbury patients turn to Accurate Aesthetics for nose surgery because they are unhappy that their nose is too broad or too large overall. Some people pursue rhinoplasty to improve the look of an unsightly hook at the end of their nose or a bump on the bridge of it. Still, others are frustrated by the asymmetrical quality of their nose.

Aside from these cosmetic concerns, some patients have structural problems with their nose — such as a deviated septum — that can cause chronic breathing issues. Those with structural nasal problems may snore excessively each night or be forced to breathe through their mouth during the day.

Individuals who are unhappy with the look of their nose or the way it functions might be appropriate candidates for nose surgery. With nose surgery, Dr. LoVerme can address various cosmetic and / or structural concerns, making either a subtle or dramatic adjustment to the nose, to restore a sense of balance to the facial features.

Rhinoplasty Candidacy

It is important for prospective patients who are considering undergoing rhinoplasty to come in for a consultation with Dr. LoVerme. During consultation, the facial plastic surgeon evaluates patients’ nasal anatomy and listens to their cosmetic concerns. He uses this information to establish candidacy and recommend a suitable surgical treatment plan to satisfy each patient’s unique needs and surgical goals.

However, appropriate rhinoplasty candidates generally include individuals who:

• Have an overall positive self-image, but feel frustrated or self-conscious about the shape, size, asymmetry or poor function of their nose
• Are in good general health
• Have decided to pursue rhinoplasty to satisfy a personal concern, not to satisfy someone else’s whims
• Are not currently pregnant
• Recognize both the potential complications as well as the benefits of undergoing rhinoplasty
• Are over the age of 18 (Dr. LoVerme may perform rhinoplasty for a younger patient who has parental consent, but makes this decision on a case-by-case basis.)

The Surgical Approaches to Rhinoplasty

There are two surgical approaches to rhinoplasty: the open technique and the closed.

The open approach involves positioning the incision on the strip of skin separating the nostrils, which is called the “columella.” This approach allows the surgeon to have maximum visibility and, thus, control during the procedure. The open approach also allows Dr. LoVerme to manipulate the nasal cartilage, if needed, to improve the supporting structure. The potential drawback of this technique is that there may be additional scar tissue, swelling and other considerations.

The closed approach involves positioning the incision on the inside of the nostrils, thereby eliminating any visible scars. The experienced plastic surgeon is able to accomplish most of the procedures with this approach, leaving less scar tissue and providing a faster healing process. This is the approach most often used by Dr. Loverme.

During consultation, Dr. LoVerme suggests the surgical approach that is most appropriate for the patient’s anatomical needs and aesthetic goals. For instance, patients with non-Caucasian ethnic backgrounds often have thicker skin and tend to desire increased definition throughout the tip of the nose. Dr. LoVerme may suggest the open approach for these patients to accommodate their anatomical attributes and satisfy their surgical goals.

The Rhinoplasty Procedure

Rhinoplasty is an outpatient procedure, performed under general anesthesia.

With both the open and closed approaches, the plastic surgeon utilizes the incision to gain access to the underlying nasal structure, which consists of cartilage and bone. He then augments, reduces or contours the supporting cartilage and bone to improve the size, shape, width, angle, symmetry or structure of the nose.

Once Dr. LoVerme is content with the newly sculpted nose, he closes the incisions with sutures and dresses the treatment area with gauze and bandages. He also places a splint over the nose for added protection and support while the cartilage, bone and tissues heal.

Tip Rhinoplasty

On certain occasions, just the tip of the nose is too large and the rest of the nose is fine. In these cases, Dr. Loverme is able to reduce the size of the tip using just local anesthesia and not breaking the bones of the nose. This provides less healing time and a nicer, refined nasal tip.

Revision Rhinoplasty

Patients who have undergone rhinoplasty with another surgeon often visit Accurate Aesthetics for help in correcting the results of their initial procedure because they are unhappy with the work performed by the first surgeon.

Revision rhinoplasty is a technically challenging procedure, due to the complex three-dimensional nasal structure as well as the presence of scar tissue that often forms after the first procedure. With revision nose surgery, Dr. LoVerme’s principal focus is to restore sound nasal function. He takes aesthetic concerns into consideration, but not if that means sacrificing his ability to repair the nasal structure.

During revision nose surgery, the plastic surgeon reconstructs the nose by meticulously dissecting and using the body’s natural resources. A secondary rhinoplasty often involves cartilage grafting, with the use of either septal cartilage or cartilage taken from the ear, if needed.

What to Expect During a Consultation for Revision Rhinoplasty

Given that Dr. LoVerme is not typically the cosmetic surgeon who performed the initial rhinoplasty, it is helpful if the patient can bring photos of what his or her original nose looked like to the consultation.

It is also helpful if the patient can supply the original surgical report. However, we recognize that for many patients, it has been a long time since the initial procedure was performed, so we understand if the initial surgical records are not available. If this is the case, Dr. LoVerme will formulate a treatment plan based on his own observations during your consultation and exam. He will also establish a surgical contingency plan, if necessary.

Typically, Dr. LoVerme prefers to delay the revision rhinoplasty procedure until at least one year following the initial surgery, as this is approximately how long it takes for the nose to heal completely. In certain circumstances, Dr. LoVerme can perform an initial evaluation of the patient’s nose during the early stages of the recovery period; however, he most likely will not perform the secondary procedure until the nose has fully healed.

Also during the consultation, Dr. LoVerme inquires about the reasons that first motivated the patient to pursue rhinoplasty as well as his or her current motivating factors for seeking revision nose surgery. He also asks about the patient’s concerns regarding cosmetic issues and nasal functionality. In addition, he assesses how the initial procedure affected the patient psychologically. Taking all this information and the results of his exam into consideration, the experienced plastic surgeon determines candidacy and the most appropriate treatment plan for the individual.

Recovering From Initial or Revision Rhinoplasty

Whether recovering from initial or revision rhinoplasty, patients should expect to take one to two weeks off from work or their usual routine. They will need to keep their head elevated for the first few days following the procedure, even while sleeping. The nose may feel stuffed up, and it may be necessary to breathe through the mouth for several days. The nose may also be sore, swollen or bruised, with bruising potentially extending into the eye area. Patients should not be alarmed if they look as though they have “black eyes.” All these are expected symptoms that should subside over the course of about 10 days.

For individuals recovering from revision rhinoplasty, please note that if there was an extensive degree of scar tissue following the initial procedure, it may take longer for the final results to become apparent than it did with the first surgery.

What Does Rhinoplasty Cost?

The cost of rhinoplasty differs by patient, largely depending on the complexity of the procedure and the patient’s anatomical requirements.

Note: Certain insurance plans may cover some of the cost of reconstructive nose surgery if the procedure is needed to correct an injury-induced irregularity or to address a breathing issue.
